Evidence Checklist

Use this checklist to organize evidence after an offshore or cruise injury. Do not return to a dangerous location or interfere with an investigation simply to collect evidence.

Photographs and video: scene, deck or floor condition, equipment, lighting, warnings, weather, visible injuries, damaged property, clothing, footwear, and any later change to the condition.

Documents: incident reports, written statements, job-safety analyses, work permits, tickets, booking confirmations, excursion documents, safety instructions, emails, text messages, and company communications.

Employment and wage records: pay stubs, tax forms, schedules, offshore rotations, overtime records, certifications, job descriptions, and records showing vessel assignments.

Physical items: preserve clothing, footwear, personal protective equipment, tools, or damaged personal property when those items may be relevant. Do not alter or repair them.

Digital evidence: keep original image files, voicemails, app messages, location data, and relevant social-media messages. Avoid posting new commentary about the accident or claim.

Witness and timeline information: record names, contact information, what each witness observed, and a chronological description of events before and after the incident.
